Job summary

Southwark Council is recruiting for a Community Support Service role within the social work team, responsible for ongoing support and reviews for residents in Southwark. Responsibilities

• Engage with, inform, and adapt to changing contexts that shape practice.
• Operate effectively within own organisational frameworks and contribute to the development of services and organisations.
• Inter-professional collaboration within multi-agency settings.
• Interpret and implement statutory and departmental policy in casework for individuals, families and other groups.
• Undertake trusted assessment/provision functions to promote independence and access to equipment.
• Use judgement to intervene with individuals, families and communities to promote independence and prevent harm.
• Ensure service users and carers are at the centre of decision making and have choice and control.
• Maintain Directorate records with accurate data entry and timely updates.
• Communicate effectively with people and carers irrespective of language or communication status.
• Assess implications of change on people and advocate for personalisation and choices.
• Apply value-for-money principles in service delivery in consultation with supervisor and partners.
• Work across agencies to ensure a whole-systems approach to assessments and ongoing support
