Job summary

Qualifications

• Must hold a recognised social work qualification and be registered with Social Work England.
• ASYE programme completed.
• Knowledge of safeguarding, mental capacity and DOLS/LPS legislation.

Responsibilities

• Engage with individuals, families and communities to assess needs and promote independence.
• Make and review assessments and care plans, and convert to support plans.
• Attend case conferences, reviews and represent the Council at Court when appropriate.
• Maintain accurate records and ensure data quality and timeliness.
