Job summary

City Of Doncaster Council is seeking a dedicated Supported Accommodation Officer to work with individuals in supported accommodation, helping them address barriers, build life skills and progress towards more independent living.

You will assess needs, complete support plans, coordinate referrals and placements, monitor placements and work in partnership with housing, health, social care and the voluntary sector to secure suitable accommodation and positive outcomes.

Qualifications

• Experience working with vulnerable adults desirable.
• Experience in housing, homelessness, social care or related support services desirable.
• Good communication, organisational and problem-solving skills.

Responsibilities

• Support individuals to access and sustain appropriate supported accommodation.
• Assist with referrals, assessments and placements.
• Provide ongoing support arrangements and monitor placements.
• Maintain accurate records and develop person-centred support plans.
