Job summary

Frimley Health NHS Foundation Trust in Surrey is seeking a Rehab Support Worker to help patients in their own homes, delivering high-quality personal care and rehab to promote independence.

You will work across a multidisciplinary team, complete Level 2 Apprenticeship in Care, and may progress via a Senior Healthcare Support Worker Apprenticeship. A driving licence and car access are required.

Qualifications

Committed to outstanding levels of care, you will be working as part of a multidisciplinary team which consists of Rehab Support Workers, OTs, Nurses and Physiotherapists to support patients in their own homes. This will be either to avoid hospital admissions or support patients on discharge from hospital. You will receive lots of training to help you reach your full potential in the health and care setting, including your Senior Healthcare Support Worker Apprenticeship (L3).

You will need to be confident working on your own in patients’ homes, able to elevate issues and concerns for patient safety and be happy promoting patients’ independence by utilising rehab techniques as well as providing personal care to patients who are convalescing as well as to palliative patients. Excellent communication skills are essential.

You must have access to your own car and be able to travel independently between patients’ homes across the area in a timely manner and will need business insurance.

You will be rostered 37.5hrs over five shifts per week between 8am-9pm, seven days a week.

Main duties of the job

As a Rehab Support Worker you will be supporting patients in their own homes, providing personal care to assist with their rehabilitation. You will also be supporting those on end of life care as well as their families.
