Job summary

County Durham and Darlington NHS Foundation Trust is seeking a dynamic clinician to join the Community Therapy North Team at Chester Le Street Community Hospital. The post involves independent and joint working across Durham, Chester le Street and Derwentside, including community settings and hospital visits.

The successful candidate will work with Physiotherapists, Occupational Therapists and Associate Practitioners, travelling across sites to support adults with varied functional needs.

Qualifications

• Excellent communication and teamwork within a multi-disciplinary setting.
• Ability to observe and record physical and social changes in patients.
• Ability to work independently with patients within an agreed care plan.
• Confidential handling of patient information and documentation.

Responsibilities

• Plan and organise clinical activity with appropriate time scheduling and record keeping.
• Engage in supervision with a senior member of staff.
• Maintain clinical records in line with guidelines and legal requirements.
• Ensure safe use of the physical environment and equipment resources.
• Work as a lone practitioner in the community with telephone support as needed.
• Keep up to date with mandatory training and participate in development.
• Assist colleagues with moving and handling assessments, equipment fitting, and exercise plans.
