Job summary

Acclaim Primary Care Network is seeking an experienced Advanced Clinical Practitioner to join our forward-thinking GP network in Hull. You will manage a broad range of acute and chronic conditions independently, delivering high-quality, patient-centred care within a supportive team and environment.

Qualifications

• Post qualification clinical experience.
• Experience working within multi-professional team environments.
• Familiar with interpretation of diagnostics and principles of shared clinical decision-making.
• Continuing professional development (CPD) evidence.

Responsibilities

• Assess and triage patients with the multi-disciplinary team across the PCN.
• Provide definitive treatment and make referrals to other team members.
• Manage undifferentiated conditions and identify red flags and serious pathology.
• Undertake complex decision making to inform diagnoses, investigations, and care within a broad scope of practice.
