Job summary

The Advanced Clinical Practitioner will work as an autonomous clinician within Ash Grove Medical Centre and Eggborough Surgery, providing high-quality, patient-centred care across the practice population. The post holder will be responsible for assessing, diagnosing and managing patients presenting with a wide range of acute, minor illness, chronic and complex conditions.

Qualifications

• Provide autonomous clinical assessments and management of acute, minor and complex conditions.
• Experience with paediatric and adult patients within scope of practice.
• Ability to work independently with appropriate escalation when needed.

Responsibilities

• Conduct autonomous face-to-face clinical consultations with patients presenting with acute, minor illness, chronic and complex conditions.
• Assess, diagnose and manage patients safely and effectively within the ACP's scope of practice.
• Request appropriate investigations and coordinate onward referral within clinical guidelines.
