Job summary

ClwydAlyn is seeking a Homeless Support Officer to join our Supported Living team in Conwy. You will help residents live as independently as possible, tailoring support to individual needs and goals while building confidence and practical life skills.

You will work with partner agencies to facilitate education, employment and community opportunities, maintain accurate records and promote safeguarding. This role offers meaningful daily impact within a compassionate housing charity.

Qualifications

• Experience delivering person-centred support to vulnerable individuals.
• Experience supporting people with learning disabilities, autism, mental health needs or other complex support needs.
• Knowledge of trauma-informed practice and ACEs.
• Experience promoting independent living skills such as budgeting, cooking, tenancy management, education, training or employment.
• Experience working with partner agencies, including health professionals and social care.
• Understanding of homelessness, mental health, substance misuse, VAWDASV, neurodiversity or related services.
• Strong communication and relationship-building skills.
• Experience managing risk and safeguarding concerns.
• Good IT skills and maintaining accurate records; familiarity with housing and tenancy sustainment would be advantageous.

Responsibilities

• Building positive, professional relationships with residents and providing consistent support throughout their journey.
• Developing, reviewing and delivering person-centred support plans and risk assessments.
• Supporting residents to access education, training, volunteering and employment opportunities.
• Encouraging life skills development, independence and tenancy readiness.
• Facilitating activities that promote wellbeing, confidence, social inclusion and community engagement.
• Working closely with partner agencies including housing, health, social care and support services.
• Maintaining accurate case notes, support records and incident reporting.
• Supporting safeguarding practices and promoting safety and wellbeing.
• Undertaking housing management duties, including benefits and tenancy matters.
• Contributing to a safe, welcoming and supportive environment across the service.
