Job summary

Southwark Council's Reablement Service invites a dedicated Reablement Officer to join our team and support residents to regain skills and live independently. You will deliver practical, person-centred assistance in homes and the wider community, building confidence and promoting lasting independence.

We focus on prevention, early intervention and strengths-based practice, collaborating across housing, health and community services.

Qualifications

• Experience delivering frontline support in housing, social care or community services.
• Strong people skills and an outcome‑focused approach.
• Ability to work independently and manage changing priorities.

Responsibilities

• Provide practical, person-centred support to residents in homes and community.
• Create personalised reablement plans to support independence.
• Help residents develop confidence and life skills for independent living.
• Support residents moving from supported accommodation or facing tenancy risks.
• Coordinate proactive community-based support and prevent homelessness.
• Manage a caseload and monitor progress to improve outcomes.
• Build partnerships with housing officers, landlords, health professionals and community services.
• Record progress accurately and contribute to service improvement.
