Job summary

Milton Keynes City Council is recruiting a Best Start Inclusion Practitioner to join our Best Start Family Hubs team. You will identify emerging special educational needs and provide timely advice, guidance and practical support to families, helping children achieve the best possible start in life.

Based across family hubs and community settings, you’ll coordinate plans, deliver early interventions and work with health and education partners to connect families with the right services.

Qualifications

• A Level 3 qualification or equivalent in early years, education, health, or related field.
• Experience supporting children and families across professionals.
• Ability to build trusting relationships with families.
• Experience in assessing needs and developing and monitoring support plans.
• Understanding of inclusive practice and local SEND services.
• Ability to manage workload in a busy community setting.

Responsibilities

• Deliver high-quality support to children and families by identifying emerging needs and providing practical early intervention.
• Support the development of inclusive approaches through activities, guidance and tailored support.
• Lead on a caseload of families with emerging SEND, coordinating plans and acting as Lead Practitioner where appropriate.
• Collaborate to connect families with health, education and community services.
• Ensure assessments use observations and recognised tools to identify needs and inform plans.
• Promote inclusive practice across Best Start Family Hubs and partner services.
