Job summary
Care Dorset Ltd is seeking a Community Reablement Worker to deliver high-quality, person-centred reablement in the Purbeck area. You will support individuals at home, helping them regain independence and maintain wellbeing with coordinated care from a multidisciplinary team.
The role requires delivering daily living assistance, medication administration per guidelines, and ongoing assessments. Excellent communication and safeguarding awareness are essential, with weekend shifts and flexible
Qualifications
- Experience delivering personal care and reablement in community/homecare settings.
- Ability to deliver person-centred care and promote independence.
- Strong communication with service users, families and colleagues.
- Ability to assess needs and plan appropriate care and support.
Responsibilities
- Provide reablement support in clients’ homes to promote independence.
- Develop and implement person-centred care plans.
- Conduct initial assessments and regular care reviews.
- Collaborate with occupational therapists, nurses and social workers to deliver holistic care.
- Monitor progress and document wellbeing; report concerns to management.
