The Continuing Care Support Worker role is based within the Northern Health and Social Care Trust, delivering 24 Hour Home Nursing care under the guidance of a Registered Nurse in the Ballymena area. It involves working in shifts to provide compassionate, person-centred care to service users in their own homes, with induction and competency requirements completed prior to practice.

The role supports patients through daily living activities, prepares for emergencies, and ensures confidentiality

Responsabilités

• Maintain confidentiality of information regarding patients/service users at all times.
• Respect the dignity, wishes and beliefs of patients/service users.
• Obtain consent for all care and treatment provision.
• Assist with daily living activities as delegated by registered staff.
• Undertake clinical duties as required within care package and induction.
