Essex Partnership University NHS Foundation Trust is recruiting for a role in an older adult inpatient ward. You will assist with implementing individual care plans and work under supervision of a qualified nurse within a multidisciplinary team to deliver 24/7 care.

You will engage with patients and carers, provide Activities of Daily Living support, and participate in de-escalation and safeguarding when needed. The post offers supportive supervision and access to wellbeing services.

Qualifications

• Assist with care plans for older adults.
• Work within an MDT to deliver nursing care.
• Be flexible with shifts and supervision requirements.

Responsabilités

• Maintain effective relationships with patients and carers.
• Implement prescribed nursing care considering holistic needs.
• Record outcomes of care plans in the required format.
• Assist with management of patients exhibiting challenging behaviour.
