Job summary

Thames Reach is a charity in London working to end homelessness. We are looking for a Support Worker to join the Robertson Street Team, providing people-centred support in our hostels and helping them access health, housing and learning opportunities.

You will work in a 24-hour team, conduct risk and needs assessments, make referrals to external services, and support client moves out within set timeframes while maintaining a safe, respectful environment.

Qualifications

• Experience and understanding of homelessness support is desirable.
• Ability to conduct risk and needs assessments.
• Capable of liaising with external agencies and councils.

Responsibilities

• Contribute confidently to day-to-day service delivery of a fast-paced hostel.
• Conduct and produce detailed Risk Assessments, Needs Assessments and move-on plans with clients.
• Make effective referrals and liaise with external agencies and councils.
• Help increase numbers of client engagements and moving out within a set timeframe.
• Ensure the environment is safe, healthy, and reflective of our ethos and supports our clients.
• Be outcome focussed and contribute to high performance and continual improvement.
