Job summary

Perth & Kinross Council is recruiting a Social Work Assistant in the Early Intervention and Prevention service. The role covers North and South Perthshire and Perth City, requiring a driving licence for travel.

36 hours per week, with opportunities for training and professional development within a supportive team. The successful candidate will carry out assessments, plan and review care packages under SDS, and work with partners to help adults age 16+ live more independently.

Qualifications

• Strong assessment skills and understanding of SDS.
• Ability to integrate and collaborate across teams and services.
• Knowledge of PKC services, demographics, and adult support policies.
• Must have a valid UK driving licence for travel across Perth and Kinross.

Responsibilities

• Carry out Outcome Focused Assessments for adults and carers.
• Negotiate flexible packages of care and monitor individual care plans.
• Review care packages delivered via Self Directed Support (SDS).
