Job summary

Ability Housing Association in London is seeking a dedicated Floating Support Worker to join our Hillingdon team. You will deliver housing-related support to adults with mental health issues in their homes and community, helping them develop independent living skills and access health services.

You will work with 13 customers using a strength-based approach, maintain SMART support plans and carry out risk assessments, with a focus on independence and preventing hospital admissions.

Qualifications

• Full UK driving licence and access to a car essential.

Responsibilities

• Deliver housing related support to customers in their own homes and in the community, including helping with independent living skills, benefit maximisation and accessing health appointments.
• Support customers with finding and securing alternative accommodation.
• Assist customers in setting up and maintaining a home/tenancy, including maintenance, rent payments and being a good neighbour.
• Work with 13 customers using a strength-based, person-centred approach and SMART support plans; ensure plans and risk assessments are up to date.
• Help minimise unplanned hospital admissions by providing emotional support and signposting to health services.
