Job summary

Confidential in Hillingdon is hiring a support worker for the Floating Support Team. The role involves delivering housing related support in customers’ homes and the community, helping with independent living and accessing health services.

You will work with 13 customers using a strength-based, person-centred approach, co-producing SMART support plans and ensuring plans and risk assessments are up to date. A driving licence and access to a car are essential.

Qualifications

• 40-hour per-week support worker role.

Responsibilities

• Deliver housing related support to customers in their homes and in the community.
• Support with finding and securing alternative accommodation.
• Assist customers in setting up and maintaining a home/tenancy, including rent and arrears.
• Develop and review SMART support plans with customers to enable independence.
• Minimise unplanned hospital admissions by providing emotional support and signposting to health services.
