Job summary

Hampshire County Council is seeking a Best Start Inclusion Practitioner to ensure young children with emerging or identified needs access the right support early. You will work across the Best Start Family Hubs network, champion inclusion and help improve developmental outcomes for 0-5s.

You'll collaborate with health, education and SEND services, build partnerships with families and professionals, and support timely, inclusive interventions that reduce inequalities and strengthen family

Qualifications

• Level 3 qualification in early years, education, health, SEND or related field.
• Experience with children aged 0-5 with SEND and their families.
• Experience of multi-agency working across education, health and family services.
• Knowledge of early child development, early intervention, inclusive practice and SEND pathways.
• Excellent communication and relationship-building skills with professional judgement.

Responsibilities

• Provide direct support, advice and interventions to families with children aged 0-5 with SEND.
• Use observation and screening tools to identify needs early and plan support.
• Coordinate with health, education and SEND partners to improve outcomes.
• Promote inclusive practice across Best Start Family Hub settings and access to early education entitlements.
• Maintain records, monitor progress and ensure safeguarding and information governance compliance.
