Hampshire County Council is seeking a Best Start Inclusion Practitioner to support 0-5 children with emerging or identified additional needs. You’ll work across the Best Start Family Hubs network, champion inclusion and help ensure every child has the best possible start in life.

You’ll collaborate with health, education and SEND professionals, provide direct support to families, and use evidence-based approaches to plan early interventions while safeguarding and information governance

Formación

• Level 3 qualification in early years, education, health, SEND or related field.
• Experience with children aged 0-5 with SEND and their families.
• Experience of multi-agency working across education, health and family services.
• Strong knowledge of child development, early intervention and SEND pathways.
• Excellent communication and interpersonal skills; ability to work independently.

Responsabilidades

• Provide direct support and interventions to families with 0-5s with needs.
• Identify needs early using observation and screening tools.
• Coordinate with health, education and SEND partners to improve outcomes.
• Promote inclusive practice and access to early education entitlements.
• Maintain records and ensure safeguarding and information governance.
