Job summary

Hampshire County Council is seeking a Best Start Inclusion Practitioner in Eastleigh to support 0-5s with identified needs across the Best Start Family Hubs network. You will work with health, education and SEND partners to ensure timely and inclusive support, promoting early intervention and better developmental outcomes.

Responsibilities

• Provide direct support and interventions to families with 0-5s with emerging or identified needs.
• Use evidence-based observation to identify needs and plan support.
• Collaborate with health, early years, schools and SEND to coordinate services.
• Promote inclusive practice across the Best Start Family Hub network and early education entitlements.
• Maintain accurate records and ensure safeguarding and information governance compliance.
