Job summary

Waythrough is seeking a Complex Needs Practitioner to join the One Recovery Buckinghamshire (ORB) service in Aylesbury. You will manage your own caseload and deliver psychosocial interventions to people with complex needs related to substance misuse.

Working with health professionals, social care, housing, and community organisations, you will support individuals to regain independence, improve wellbeing and sustain recovery.

Qualifications

• Experience with complex needs and substance misuse is essential.
• Experience working with adults in addiction or related services is preferred.
• Strong communication and case management skills are required.

Responsibilities

• Manage a diverse caseload of individuals with complex needs related to substance misuse.
• Build trusted, supportive relationships with service users and, where appropriate, their families.
• Complete comprehensive assessments, care plans, psychosocial interventions, reviews, and aftercare support.
• Develop personalised, strengths-based recovery plans focused on achieving meaningful outcomes.
• Identify, assess, and manage risk, including safeguarding concerns.
• Work closely with partner agencies to coordinate timely, effective, and holistic support.
• Advocate on behalf of service users, helping them access healthcare, housing, social care, education, employment, and wider community services.
• Promote recovery, independence, wellbeing, and harm reduction through evidence-based interventions.
• Maintain accurate and confidential records in line with GDPR and organisational policies.
• Participate in multidisciplinary meetings, case discussions, and continuous service improvement.
