Job summary

Leicestershire County Council is recruiting a Community Support Worker to meet with adults, carers and families, assess their needs under the Care Act 2014 and create plans to access support. You will conduct home visits and work in various settings with supervision to deliver person-centred care.

You will handle safeguarding, mental capacity and best-interest considerations, with opportunities to progress into apprenticeships and leadership routes.

Qualifications

• Relevant health/social care qualifications such as Level 2/3 in care.
• Maths and English Level 2 or equivalent for documentation and assessments.
• Experience in social care settings and collaboration with other agencies.

Responsibilities

• Meet individuals, families and carers to assess needs under Care Act 2014.
• Devise plans to access support, connect with local community or refer to agencies.
• Identify safeguarding concerns, mental capacity, and high-risk situations.
• Undertake home visits, hospital or client settings with office base support.
• Manage caseload and work towards clear objectives.
