Job summary

Leicestershire County Council is seeking a Community Support Worker to join the Mental Health Team covering Blaby, Oadby & Wigston, and Harborough areas. The role involves visiting clients in their homes or other settings, assessing needs under the Care Act 2014, and coordinating support plans with partners to promote independence.

You will support individuals, families and carers, undertake risk assessments, and report safeguarding concerns as needed.

Qualifications

Hold a Level 2 in Maths and English and a relevant health/social care qualification.
Have Level 2 or 3 qualification in adult social care (e.g., NCFE CACHE Level 2 Diploma in Care).
Experience working with adults in a social care setting and collaboration with other agencies.
Understanding need and risk assessments and the issues/needs of adults and families/carers.
Strong communication and relationship-building skills.
Able to manage a caseload and work to clear objectives.

Responsibilities

Meet with individuals, families and carers to assess needs under the Care Act 2014.
Devise a plan to access required support, including community connections and referrals.
Identify safeguarding concerns and report where necessary.
Undertake visits at home, in hospital or other locations comfortable for the person.
Work within a multi-disciplinary Mental Health team.
Contribute to enabling independence and resilience in service users.
