Job summary

Responsibilities

• Provide specialist assessment, treatment, education, and support for patients with IBD.
• Manage nurse-led telephone and email advice services, ensuring timely access to specialist care and support.
• Coordinate outpatient care, patient follow-up, and monitoring of symptoms, investigations and treatment response.
• Monitor patients receiving biologic and advanced therapies, ensuring safe, effective and evidence-based treatment.
• Identify and manage disease flare-ups promptly to reduce complications and admissions.
• Promote patient self-management, education and long-term health outcomes.
• Support discharge planning, care coordination and continuity of care across services.
• Collaborate with consultants, pharmacists, dietitians, primary care teams and MDTs.
• Ensure compliance with NICE guidance and national IBD standards.
