Job summary

Frimley Health NHS Foundation Trust is seeking an experienced Clinical Specialist Nurse to join the Gastroenterology team at Wexham Park Hospital. You will provide expert assessment, education and support for people with IBD, coordinating care across MDTs and ensuring high-quality patient-centred services.

You will lead specialist advice via clinics and remote contacts, monitor biologic therapies, and contribute to service development, audit and evidence-based practice in line with NICE guidance.

Qualifications

• Provide specialist assessment, treatment, education and support for IBD.
• Manage nurse-led telephone and email advice services.
• Coordinate outpatient care and ongoing monitoring of symptoms.
• Monitor patients on biologic/advanced therapies.
• Promote self-management and patient empowerment.

Responsibilities

• Lead on disease management and timely access to specialist care.
• Collaborate with consultants, pharmacists and dietitians in MDT.
• Ensure NICE guidance and national IBD standards are followed.
• Develop guidelines, pathways and best practice in gastroenterology.
• Support discharge planning and care coordination across settings.
• Contribute to service development, audits, and staff education.
• Respond to increasing service demand and optimise patient flow.
• Support education and dissemination of evidence-based practice.
