Job summary

Medway Council is seeking an experienced social worker in Adult Social Care to join the Early Help and Prevention team. You will review care packages, work in a strengths-based, person-centred way, and contribute to outcomes for adults with social care needs.

The role is hybrid, with time split between Gun Wharf and home, and offers a clear CPF-based progression with competitive pay. You will be registered with Social Work England and have completed ASYE.

Qualifications

• ASYE completed and registered with Social Work England.
• Qualification in social work and current registration is essential.
• Experience reviewing packages of care to ensure value and outcomes.

Responsibilities

• Review packages of care to ensure best value and positive outcomes.
• Work within strengths-based, person-centred approach to care planning.
• Support transformation and develop local links with communities and partners.
