• Support Practices to provide clinical tools and resources for long-term conditions.
• Ensure clinical services are delivered within protocols and operating instructions.
• Identify opportunities for clinical reviews at practice, PCN or ICB levels.
• Enhance awareness of Interface Clinical Services within primary care and NHS relationships.
