Job summary

Transform Community Development is seeking a Housing Support Worker to develop and deliver high-quality support promoting participant choice, control and independence. You will work with vulnerable participants to help them secure tenancy and make positive life choices, with a focus on empowerment and advocacy.

The role operates on a 12-hour shift system, including day and night shifts, and requires commitment to the wellbeing and progression of participants within the homelessness service.

Qualifications

• Experience in housing or social care settings preferred.
• Ability to manage time, prioritise tasks under pressure.
• Strong communication and advocacy with participants.

Responsibilities

• Liaise with Local Authority for referrals of participants.
• Interview participants to assess suitability and action decisions.
• Assess support needs and risk, compile plans with the participant.
• Ensure plans and risk assessments are implemented and reviewed.
• Connect participants with other support agencies as needed.
• Attend MDT meetings with partner agencies.
• Support participants to appointments and advocate for them.
• Assist participants with budgeting and daily tasks.
• Comply with statutory guidelines including SSSC and Care Inspectorate.
