Qualifications

• Experience managing caseloads of vulnerable residents with complex needs.
• Ability to conduct home visits and holistic assessments to draft and review support plans.
• Knowledge of safeguarding procedures and welfare checks.

Responsibilities

• Case management of a caseload of vulnerable residents with complex needs.
• Carry out home visits and holistic assessments to draft and monitor support plans.
• Provide guidance on Universal Credit, Housing Benefit and debt management.
• Conduct welfare checks and raise safeguarding referrals when risks are identified.
• Collaborate with health services, social care, training providers, and voluntary organisations.
