G2 Recruitment Group Limited is seeking a dedicated Case Manager to support vulnerable residents in the Greater London area. You will manage a caseload with complex personal needs, mental health concerns, and safeguarding considerations, delivering person-centered support and coordinated care.

This role involves home visits, holistic assessments, and drafting tailored action plans, plus guidance on Universal Credit, housing benefits, and debt management.

Qualifications

• Experience managing caseloads of vulnerable residents with complex needs.
• Ability to conduct home visits and holistic assessments to draft and review support plans.
• Knowledge of safeguarding procedures and welfare checks.

Responsabilités

About the Role

Responsibilities

• Case Management: Manage a caseload of vulnerable residents with complex personal needs, mental health issues, or safeguarding concerns.
• Assessments & Support Plans: Conduct home visits and holistic assessments to draft, monitor, and review personalized support action plans.
• Income Maximization & Benefits: Provide specialist guidance on Universal Credit, Housing Benefit, Discretionary Housing Payments, and debt management.
• Safeguarding & Welfare: Perform regular welfare checks, identify risks of domestic abuse or exploitation, and submit appropriate safeguarding referrals.
• Multi-Agency Working: Build and maintain networks with health services, social care, employment training providers, and voluntary organizations.
