Job summary

The Hospital to Home Service provides short-term, person-centred support to promote recovery and enable safe and timely discharge from hospital.

The Assistant Coordinator plays a key supporting role in the delivery of the service on Cedar Ward, Petersfield by engaging with patients to support their wellbeing, recovery, and discharge planning and by coordinating activities, volunteers, and follow-up support.

Qualifications

• Experience of working with or supporting older people.
• Strong communication and interpersonal skills are essential.
• Ability to work independently and as part of a team.

Responsibilities

• Support the Senior Coordinator in day-to-day delivery of the Hospital to Home service.
• Engage with patients to support wellbeing, recovery, and discharge planning.
• Assist in coordinating activities, volunteers, and follow-up support.
• Help ensure patients are connected to community services on discharge.
• Maintain accurate records and support data collection and reporting.
