Job summary

Kent County Council is recruiting an Arranging Support Hospital Discharge Team Coordinator to help people leave hospital safely. You will manage referrals, source placements and negotiate with providers to secure best value, while explaining care funding arrangements and assisting families with choices and costs.

The role involves working with Health colleagues and Locality Teams to deliver high-quality, person-centred outcomes and to support timely discharges from hospital.

• Managing and prioritising referrals
• Sourcing placements and negotiating with providers to achieve best value
• Explaining care funding arrangements and undertaking light-touch financial assessments
• Supporting individuals and families to understand options and costs
• Collaborating with Health colleagues, Locality Teams and partners to enable safe and timely hospital discharges
