Warwickshire County Council is seeking a qualified social worker for its Hospital Team. You will manage a caseload of older adults, undertake robust assessments, and develop care and support plans while travelling across Warwickshire as required.

You will work within a multidisciplinary team and be part of a dynamic, supportive environment. You must hold a HCPC-registered Social Work qualification, have IT proficiency, and a valid driving licence for post-discharge reviews in care homes.

Formación

• Professional Social Work qualification and HCPC registration are essential.
• Must be able to work 5 days over 7, with weekends on a 4-week rota.
• Duties include multidisciplinary team collaboration, meetings and reviews.
• Driver's licence is essential for post-discharge reviews, especially in care homes.
• Ability to think flexibly with a strength-based approach and work under pressure.
• Experience in adult social care and good IT skills required.
• Hospital social care experience desirable; willing to learn in a fast-paced environment.
• Fluency in spoken English to provide advice to the public.
