The Hospital Social Care Team at Warwickshire County Council is recruiting a Level 2 Social Worker for full-time work based at the Hospital of George Eliot Hospital in Nuneaton and Rugby St Cross Hospital. You will manage a caseload of older adults in acute hospital settings, undertake robust assessments and care planning, and collaborate with a wide range of health and social care partners.

Qualifikationen

• Professional Social Work qualification and HCPC registration.
• Able to work 5 days over 7 with a 4-week weekend rota.
• Work closely with Multidisciplinary Health & Social Care teams.
• Participate in team meetings and receive supervision.
