We are seeking an empathetic and driven Senior Social Worker to join our Short Term Support Service in Morecambe Bay on a full time, permanent basis.

Key Responsibilities

  • Provide a single point of access to community intermediate care services, including reablement and residential rehab.
  • Support hospital discharges and work with the wider multi-disciplinary team to prevent hospital admissions.
  • Utilize the Discharge to Assess model for assessments away from the hospital environment.
  • Conduct holistic, face-to-face Care Act Assessments, reviews, and support planning using a strengths-based approach.

  • Registration with Social Work England (SWE) or application to register with SWE.

  • A car for use in connection with the duties of this post, insured for business use. Consideration may be given if you cannot drive due to a disability.
