Job summary

Leaders in Care is seeking an Adult Hospital Discharge Social Worker in the United Kingdom. The role offers an up-to £40 per hour rate and two days per week of home-working with flexible hours to support better work-life balance.

You will help patients discharged from hospital access services and resources, coordinating with families and healthcare teams to create ongoing care plans that enable safe returns home and continued community support.

• You'll be responsible for ensuring that patients who are being discharged from the hospital are able to access the services and resources they need to successfully transition back to their homes and communities. You'll work closely with patients, their families, and other healthcare providers to assess their needs and develop a plan for their ongoing care.

I'm currently looking for an Adult Hospital Discharge Social Worker paying up to £40 per hour!

You will also be awarded the opportunity to work from home two days per week and the chance for flexi-hours, giving you that extra time to help improve your work-life balance.

You'll be responsible for ensuring that patients who are being discharged from the hospital are able to access the services and resources they need to successfully transition back to their homes and communities. You'll work closely with patients, their families, and other healthcare providers to assess their needs and develop a plan for their ongoing care.
