Job summary

Leaders in Care is recruiting a locum Social Worker to join an office-based hospital discharge team in the United Kingdom. You will assess, plan and review care needs for adults leaving hospital, coordinating with multidisciplinary colleagues to ensure safe, timely discharges.

You will work in a fast-paced setting, providing person-centred support and building strong relationships with patients and carers. Competitive hourly rates and dedicated consultant support are provided.

Qualifications

• Social Work qualification.
• Current registration with Social Work England.
• Experience within adult social care, hospital discharge or Transfer of Care services.
• Strong assessment, care planning and review skills.

Responsibilities

• Complete assessments within 14 to 28 days.
• Carry out six to 12-week reviews.
• Contribute to a light-touch triage process.
• Work closely with healthcare colleagues to ensure smooth transitions from hospital to home.
• Promote independence and reduce avoidable readmissions.

The role

You’ll work within a dynamic hospital discharge service, supporting people across a 200-bed pathway to return home safely with the right package of care in place. The service provides an effective route for re-referral where longer-term support is needed.

You’ll complete assessments within 14 to 28 days, carry out six to 12-week reviews and contribute to a light-touch triage process. Working closely with healthcare colleagues, you’ll help ensure smooth transitions from hospital to home, promote independence and reduce avoidable readmissions.
