We are looking for an experienced, talented, and compassionate Hospice Social Worker for a full-time position.
The Hospice Social Worker will work as a part of a home-based interdisciplinary team providing counseling and targeted interventions for a population of homebound patients and their caregivers who are receiving Palliative or Hospice services through NVNA and Hospice. You will conduct an initial psychosocial assessment of the patient/caregiver contributing to the Agency's plan of care. The Hospice Social Worker strives for optimal patient outcomes with improved functioning and an increased ability to self-manage their bio-psych-social challenges. To provide medical services to Hospice patients and/or family members reflective of the philosophy, mission, and standards established by the Agency.
