A leading local authority in Cardiff seeks to hire a Social Worker to join the Cardiff and Vale Health Inclusion Service. The position focuses on supporting vulnerable groups facing social and health inequalities. This role will engage with individuals across various community settings, conducting assessments and managing care packages to enhance wellbeing and independence.

This post offers an exciting opportunity to be part of a developing service aimed at reducing health and social inequalities for individuals who face significant barriers when accessing traditional services.
The Cardiff and Vale Health Inclusion Service (CAVHIS) has been evolving since 2020 and has recently expanded its remit to include new models of care for broader health inclusion groups. These groups specifically include asylum seekers, vulnerable refugees, people experiencing homelessness, sex workers, individuals leaving prison, and mobile Gypsy Travellers. CAVHIS delivers these specialist primary care services across various community settings, such as homeless hostels, as well as the Emergency Unit and University Hospital Wales.
This role will involve conducting assessments with individuals within the identified health inclusion groups, commissioning outcome-focused care and support packages, and undertaking direct work with individuals.
While the role will collaborate with CAVHIS and partner agencies, it will be managed by the Adult Services Team Manager responsible for Specialist Support Services. These services are committed to strengths-based social work practice and working collaboratively with individuals to meet their identified outcomes and maximize their independence and wellbeing.
The teams are also dedicated to working in a trauma-enhanced manner with individuals, and you will have the opportunity to develop these skills alongside our strengths-based practice. Our Social Workers are valued for the skills and knowledge they bring to the service. They are key to the continued practice of strength-based social work approaches in Cardiff.
The role will work alongside CAVHIS, engaging with individuals within the identified health inclusion groups who have care and support needs. As a Social Worker, you will undertake assessments, draw up care and support plans with individuals identifying their outcomes and commission care and support. You will also be required to manage a busy caseload.
As a Social Worker you will receive regular supervision and when appropriately qualified be expected to supervise unqualified team colleagues. You will have a role in supporting the continued implementation of strength-based practice and of contributing to Quality Assurance in Adult Social Services.
The team provides advice and support to other teams regarding working with those who are homeless and have care and support needs in the city. You will be expected to develop your knowledge of the issues which affect those who are homeless and local services. You will also be supported to use evidence-based practice with individuals.
1. Experience of working with adults and undertaking assessment with those individuals who require support services.
2. Commitment to excellent practice with individuals, families, Carers and Communities.
3. Assessment skills and a clear understanding of the Social Services and Wellbeing Act (Wales) 2014.
4. Confidence and knowledge in operating within the Mental Capacity Act legal framework. Experience of Court of Protection work and the process is an advantage.
5. Commitment to safeguarding and promoting the wellbeing of citizens is an expectation of all who work in the service area.
6. A commitment to working closely with colleagues in an established specialist social work team and to multi-professional working with colleagues in statutory and third sector services.
7. Possess knowledge of, or demonstrate a willingness to develop knowledge of, asylum and immigration systems, as well as homelessness systems and services.
8. The ability to speak other languages, in particular Welsh, and Community Languages would be an advantage.

With effect from the 1st April 2025 this post attracts a market supplement of
£3000 per annum full time equivalent.
This payment is reviewed on a 12 monthly basis.
