St Leonard's Hospice is seeking a full-time Registered Nurse to join the Hospice@Home team, delivering high-quality community-based palliative care and symptom management. The role includes a shift pattern across days, evenings, weekends and bank holidays, offering a rewarding path to develop palliative care skills.

You will work with a multidisciplinary team, provide compassionate care, maintain records, and support families in a home setting.

Qualifications

• When applying, you should have experience as a nurse in community settings with strong nursing skills and medicines management.
• You will need excellent communication skills and be empathetic and patient-centred.
• You should be able to respond to emotionally challenging situations with professionalism.
• You must be motivated, dynamic and enthusiastic about hospice care.

Responsibilities

• Provide skilled, compassionate nursing care and symptom management to community patients as part of Hospice@Home.
• Plan, deliver and evaluate the effectiveness of care for patients.
• Offer emotional support to patients' loved ones.
• Maintain accurate written records and communicate with the team.
• Create a safe and positive environment for patients.
