Job summary

St Michaels Hospice in the North Hampshire area is seeking a highly motivated nurse to join the Hospice at Home Team, specialising in end-of-life care and supporting patients at home in their final weeks and days. The role offers opportunities to rotate between inpatient and home-care services, with mandatory training and ongoing supervision.

Qualifications

• Substantial post registration experience supporting patients and relatives through the dying process.
• Experience of working with or within an MDT setting.
• Experience of developing and teaching others.
• Full UK driving license and access to transport for community visits.

Responsibilities

• Provide specialist palliative care in the home, with 24/7 nursing cover.
• Assess, plan, implement and evaluate care plans with patients and families.
• Mentor, supervise and support junior colleagues and students.
• Develop education and training for professionals involved in palliative care.
• Work collaboratively with the wider multidisciplinary team to ensure seamless care.
