A leading organization in social support services seeks a dedicated individual to assist homeless individuals, enhancing their lives through effective advocacy and partnership coordination. The role requires strong communication, initiative, and knowledge of mental health interventions. Establish effective communication and referral pathways for individuals who are homeless and in contact with other health and care agencies.
Attend relevant multi-disciplinary team meetings to support links to health and social care services, peer support networks, community assets, and mutual aid.
Coordinate the transfer of any accommodated out-of-area individuals.
Increase and enhance partnership working with primary care, mental health services, housing, and other voluntary organizations supporting rough sleepers.
Provide advocacy for access to partnership services.
Work with service users to support social (re)integration, enabling them to lead meaningful and purposeful lives: promoting recovery, resilience, peer support, and self-determination.
