Qualifications

• Provide care to patients via home visiting service under GP supervision.
• Carry out focused history taking, physical examinations and establish management plans.
• Work with the primary care team to deliver patient-centred care and follow approved protocols.
• Participate in clinical audits, significant event reviews and other quality improvement tasks.

Responsibilities

• Provide care to patients via home visiting service.
• Provide first point of contact care with history taking and clinical assessment to form diagnoses and management plans.
• Support management of patients with conditions through clinics such as family planning, COPD, diabetes, anticoagulation (as trained).
• Refer patients to other services using appropriate referral pathways.
• Ensure safe handover of care within and outside the practice.
