Job summary

Rotherham Doncaster and South Humber NHS Foundation Trust in Doncaster is recruiting a Band 5 Home Treatment Practitioner to join the Doncaster Home Treatment Team. The post is on site and covers 37.5 hours per week with shifts from 08:00 to 21:30, seven days a week.

You will provide short-term intensive community support for adults with acute mental health needs to prevent hospital admission, with full nursing assessment, care planning and delivery, and collaboration with crisis services.

Qualifications

• Registered Mental Health Nurse with current professional registration (NMC) or registered allied health professional.
• Evidence of Continuing Professional Development (CPD).
• Key skills in literacy, numeracy and IT Level 2 or equivalent.
• Risk assessment and management training.
• Supervision training.

Responsibilities

• Provide a clinical service that is patient-focused, accessible and responsive on a 7-day-a-week basis.
• Triage mental health referrals to determine priority and pathway.
• Deliver nursing care including full mental health assessment, care planning and delivery.
• Refer to the most appropriate team/service when indicated.
• Work with crisis house to lead on admissions as a service.
• Facilitate early discharge from inpatient wards when clinically indicated.
• Deliver home-based treatment interventions in line with best practice.

37.5 hours – 08:00 to 21:30 365 days a year

An exciting opportunity has arisen for a band 5 nurse to join Doncaster intensive Home Treatment Team. We provide short-term intensive support in the community to individuals with acute mental health needs to prevent hospital admission.

We provide a service between 08.00 and 21.30 7 days per week and staff are expected to work shift patterns to cover this. Most team members work 3 long shifts, which helps promote a good work life balance.

The team works in partnership with third sector and peer organisations to deliver person centred crisis care, therefore transparency and openness is vital.

Main duties of the job

• To provide a clinical service that is patient focussed, accessible and responsive on a 7-day a week basis.
• To triage all mental health referrals into the Team to determine the priority for service response and the appropriate clinical pathway.
• To provide and deliver highly skilled nursing care, including full mental health assessment, care planning and care delivery, biopsychosocial interventions, clinical risk assessment and management, and promoting recovery and inclusion.
• To recommend and refer, where treatment is indicated, to the most appropriate team/service.
• To work collaboratively with the crisis house to lead on care for admissions as a service.
• To facilitate early discharge from the inpatient wards where clinically indicated.
• To deliver home based treatment interventions for all patients, ensuring best practice is implemented .
