Rotherham Doncaster and South Humber NHS Foundation Trust is recruiting for a Crisis Resolution and Home Treatment Practitioner (NHS AfC Band 6). The role is full-time, permanent, and requires 37.5 hours per week with a 24/7 rota.

You will work in the crisis team providing urgent assessments, triage, and safe, person-centred care planning across shifts. The post involves multidisciplinary collaboration with nurses, psychology, and other colleagues, and offers opportunities for cross-cover within

  • Registered Mental Health Nurse, Social Worker or OT with appropriate registration.
  • ENB 998 or mentor qualification or willingness to complete.
  • Evidence of post-registration training.
  • Safeguarding training.
  • Non-medical prescribing.

Responsibilities

  • Carry out full assessments including risk assessments.

Crisis and Home Treatment Practitioner

Job overview
  • The service triages and assesses all urgent and emergency referrals to identify the most appropriate service to meet individual need when persons are experiencing a mental health crisis. This involves telephone triage, support and signposting and face to face full needs assessment care planning and referring on to relevant services.
  • The team works closely with the Home Treatment Team, and there may be opportunity for cross cover, but the primary role is within the crisis team.
  • The team currently consists of Mental Health Nurses, a Team Manager and a Clinical lead. We have in-put form Consultant Psychiatrists and a Psychologist and would welcome Occupational therapy and Social work applicants, with relevant experience and appropriate registration
  • The team provide a service over a 24/7 period encompassing a varied shift pattern.
Main duties of the job
  • To provide a clinical service that is service user focussed.
  • To triage all mental health referrals to the Crisis Team to determine the priority for service response and the appropriate clinical pathway.
  • To carry out full assessments of needs including comprehensive risk assessments of persons referred or self -referred to the service.
  • To recommend and refer, where treatment/intervention is indicated, to the most appropriate team/ service utilising all available resources.
  • To identify where appropriate treatment-based interventions for all service users ensuring that best practice is implemented.
  • To act as gatekeeper to informal admissions to hospital.

Qualifications
  • Registered Mental Health Nurse, Social Worker or OT
  • ENB 998 or other NMC approved mentor qualification or mentor qualification appropriate to discipline- or willingness to complete.
  • Evidence of post registration training
  • Safeguarding training
  • Non-medical prescribing
Experience
  • Experience of assessing and managing risk
  • Professional experience of working in acute adult mental health
  • Experience of MDT working
  • Demonstrable post registration experience as an RMN/ SW
  • Skills in coordinating and planning inter-agency approaches in mental health care
  • Experience of working in the community
  • Experience of working with statutory and nonstatutory agencies.
  • Experience of working at Band 6 level
  • Experience of managing risk Experience of delivering evidence-based treatments.
Knowledge
  • Ability to use initiative and work without immediate supervision
  • Ability to demonstrate an understanding, empathic approach
  • Skills in co-ordinating and planning interagency approaches in mental health care
  • An understanding of current mental health issues
  • Knowledge and understanding of the legislative framework for mental health services, particularly the Mental Health Act
  • Knowledge and understanding of the roles of other professionals working within mental health

  • Ability to work as part of a multi-professional team
  • Understanding of the CPA process
  • Understanding of the importance of user/carer involvement in care planning
  • Experience of Safeguarding
