Job summary

Rotherham Doncaster & South Humber NHS Foundation Trust (RDaSH) invites applications for a Band 5 Home Treatment Practitioner to join the Doncaster Home Treatment Team. We provide short-term intensive community support to adults with acute mental health needs to prevent hospital admission, across a 7-day service from 08:00 to 21:30.

The role involves triaging referrals, delivering comprehensive nursing assessments, care planning and delivery, risk assessment, and coordination with crisis

Qualifications

• Registered nurse with Mental Health Nursing (RMN) registration and valid NMC pin.
• Experience in community or home treatment settings preferred.
• Willingness to work across a 7-day service and rotating shifts.

Responsibilities

• Triage all referrals to determine priority and pathways.
• Provide and deliver skilled nursing care including assessments, care planning and delivery.
• Conduct biopsychosocial interventions and risk assessments, promoting recovery and inclusion.
• Coordinate with crisis services and refer to other teams when indicated.
• Support early discharge from inpatient wards when appropriate.
• Deliver home-based treatment interventions for all patients.

37.5 hours – 08:00 to 21:30 365 days a year

Doncaster Home Treatment Team

An exciting opportunity has arisen for a band 5 nurse to join Doncaster intensive Home Treatment Team. We provide short-term intensive support in the community to individuals with acute mental health needs to prevent hospital admission.

We provide a service between 08.00 and 21.30 7 days per week and staff are expected to work shift patterns to cover this. Most team members work 3 long shifts, which helps promote a good work life balance.

The team works in partnership with third sector and peer organisations to deliver person centred crisis care, therefore transparency and openness is vital.

Are you a passionate mental health nurse who wants to make a real difference to individuals and their families? Do you want to play an essential part in a person's recovery journey? If so, working with Home Treatment Team could be for you!

• To provide a clinical service that is patient focussed, accessible and responsive on a 7-day a week basis.
• To triage all mental health referrals into the Team to determine the priority for service response and the appropriate clinical pathway.
• To provide and deliver highly skilled nursing care, including full mental health assessment, care planning and care delivery, biopsychosocial interventions, clinical risk assessment and management, and promoting recovery and inclusion.
• To recommend and refer, where treatment is indicated, to the most appropriate team/service.
• To work collaboratively with the crisis house to lead on care for admissions as a service.
• To facilitate early discharge from the inpatient wards where clinically indicated.
• To deliver home based treatment interventions for all patients, ensuring best practice is implemented.
