Rotherham Doncaster and South Humber NHS Foundation Trust in North Lincolnshire is seeking a Clinical Nurse Specialist (NHS AfC Band 7) to lead the Crisis Resolution and Home Treatment Team with senior clinical guidance.

You will coordinate a 24/7 rota, provide clinical supervision, lead MDT reviews and drive service improvements, supporting safe discharge and effective collaboration with inpatient wards and community services.

Job overview

An exciting and challenging opportunity has arisen for a forward thinking, highly motivated, innovative, dedicated and experienced registered Mental Health Nurse to provide clinical leadership to our Crisis Resolution and Home Treatment Team in North Lincolnshire as a Clinical Nurse Specialist. As the Clinical Nurse Specialist, you will have responsibility for the promotion and assistance in the maintenance of the highest attainable standards of nursing care provided by the teams through the provision of senior clinical support and advice to the team members, ensuring you are visible to all staff. You will have the lead role of coordinating the team from a clinical perspective across a 24/7 rota, including clinical supervision, group supervision and MDT reviews. You will also work closely with the team, focusing on projects identified by management, ensuring improvement of service provision and practice. As a result, you should have demonstrable experience working as a Band 6 Nurse in an Acute Care Pathway.

Main duties of the job

We know that service users recover quickly at home, and you will be responsible for ensuring that this is done in a safe and specific way as an alternative to hospital admission. With these skills and leadership, you will be integral in ensuring that all patients under the Crisis Resolution and Home Treatment Team will have complex, detailed and meaningful care plans, risk management plans with a view to ensuring that discharge is seamless between us and community teams. You will also be responsible for working with the Crisis House and Inpatient Wards to help facilitate appropriate and timely discharge. This will involve taking the lead with both our inpatient colleagues and community-based services, ensuring that the patient is at the centre of our care.
