Job summary

Rotherham Doncaster and South Humber NHS Foundation Trust (RDaSH) is seeking a forward-thinking, highly motivated Clinical Nurse Specialist (Band 6) to lead the Crisis Resolution and Home Treatment Team in North Lincolnshire. You will provide senior clinical support and advice, ensure visible leadership, and coordinate the team across a 24/7 rota.

The post requires experience as a Band 6 nurse in an acute care pathway, delivering high-quality care plans, risk management, and seamless discharge

Qualifications

• Registered Mental Health Nurse with current registration.
• Band 6 level experience in acute care.
• Experience leading crisis teams and MDTs.

Responsibilities

• Provide clinical leadership to the CRHTT.
• Coordinate the 24/7 rota and supervise staff.
• Provide clinical and group supervision and MDT reviews.
• Collaborate with Crisis House and inpatient wards to facilitate timely discharge.
• Develop and monitor detailed care and risk management plans.
• Ensure discharge is seamless between hospital and community services.
• Be visible to all staff to promote best practice.

Job description

An exciting and challenging opportunity has arisen for a forward thinking, highly motivated, innovative, dedicated and experienced registered Mental Health Nurse to provide clinical leadership to our Crisis Resolution and Home Treatment Team in North Lincolnshire as a Clinical Nurse Specialist. As the Clinical Nurse Specialist, you will have responsibility for the promotion and assistance in the maintenance of the highest attainable standards of nursing care provided by the teams through the provision of senior clinical support and advice to the team members, ensuring you are visible to all staff. You will have the lead role of coordinating the team from a clinical perspective across a 24/7 rota, including clinical supervision, group supervision and MDT reviews. You will also work closely with the team, focusing on projects identified by management, ensuring improvement of service provision and practice. As a result, you should have demonstrable experience working as a Band 6 Nurse in an Acute Care Pathway.

We know that service users recover quickly at home, and you will be responsible for ensuring that this is done in a safe and specific way as an alternative to hospital admission. With these skills and leadership, you will be integral in ensuring that all patients under the Crisis Resolution and Home Treatment Team will have complex, detailed and meaningful care plans, risk management plans with a view to ensuring that discharge is seamless between us and community teams. You will also be responsible for working with the Crisis House and Inpatient Wards to help facilitate appropriate and timely discharge. This will involve taking the lead with both our inpatient colleagues and community-based services, ensuring that the patient is at the centre of our care.
