Job summary

Visiting service users in their homes to assess needs and create tailored support plans, while assisting with daily living activities and coordinating with health, housing and social care professionals.

Qualifications

• Experience supporting people with physical, sensory, or mental health needs.
• Strong communication skills and the ability to work both independently and in a team.
• Knowledge of NHS and social care services, and how to access them.

Responsibilities

• Visiting service users in their homes to assess needs and create tailored support plans.
• Assisting with daily living activities such as bathing, dressing, and meal preparation.
• Liaising with health, housing, and social care professionals to ensure consistent support.
• Carrying out welfare checks and helping individuals rebuild their independence.
