The role requires a full UK driving license and access to a vehicle, and offers flexible working, including remote and hybrid options, plus generous holidays and a contributory pension.

Qualifications

• Full UK driving licence and access to a vehicle.
• Basic IT skills and ability to follow procedures.

Responsibilities

• Manage a caseload to provide emotional and practical support for service users recently discharged from hospital.
• Support adults after hospital stay to recover safely at home.
• Respond to referrals from clinicians and community health professionals.
• Assess needs and follow a personalised support plan for each service user.
